ACDC Waiver of Liability

 

I hereby understand and acknowledge that the auditions, programs and events held by Atlanta Contemporary Dance Co., may expose me to many inherent risks, including accidents; falls which can result in serious injury or death; injury or death due to negligence on the part of myself or other people around me; injury or death due to improper use or failure of equipment; strains and sprains. I am aware that any of these above-mentioned risks may result in serious injury or death to myself and or my partner(s). I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class while at, or under the direction of Atlanta Contemporary Dance Co. I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.

After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and Atlanta Contemporary Dance Co., I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Atlanta Contemporary Dance Co., its officers, directors, agents, employees, organizers, volunteers, representatives, and successors from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my participation in this activity. I further agree to indemnify and hold harmless Atlanta Contemporary Dance Co., its officers, directors, employees, organizers, volunteers, representatives, and successors from liability for the injury or death of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by Atlanta Contemporary Dance Co. By my signature I indicate that I have read and understand this Waiver of Liability. I am aware that this is a waiver and release of liability, and I voluntarily agree to its terms. I understand that by participating in an audition I do so at my own risk.  I will not hold Atlanta Contemporary Dance Co., Elite Studios, and/or any members associated with either group responsible for any injury that I might sustain by participating.

ACDC Testimonial and Photo Release

 

In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to Atlanta Contemporary Dance Company and his/her agents the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to Atlanta Contemporary Dance Co. by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of Atlanta Contemporary Dance Company, and in any and all media, including without limitation Atlanta Contemporary Dance Company’s website. I acknowledge that Atlanta Contemporary Dance Company has no obligation to return any photographs or images to me. I hereby RELEASE, WAIVE and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by Proteus Fitness, including without limitation any and all claims for libel or invasion of privacy. I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.
